Permission to Skate Forms
A permission to skate form is required to try out for any organization other than the one you were registered with for the 2024-2025 season. Permission to skate forms are NOT a player release.
If you require permission to skate form please email: Jen Tomkins, Registrar ([email protected]) with the following information:
- Player Name
- 2023-2024 SJLS team
- Team and organization that player is trying out for
You will require a separate form for each team/organization you are trying out for.
Once Jen has received your email, the form(s) will be prepared for pickup. These will not be delivered, they must be picked up. Address will be included in email telling you that your PTS is ready.
Permission to skate forms do not become valid until 12:01 am on April 17, 2024.
